Change in Dismissal

In order to ensure that your child gets to their after school location efficiently and safely, we ask that they go home a set way each day.  We understand, however, that emergencies happen and there are times when you will need us to send them home a different way.  The below is our Change of Dismissal Form which you may print for this purpose.  This should be filled out whenever your child will not leave school in their usual, agreed-upon manner.  This form should be sent in with your child to give their teacher on the day of the change.  Emailing the teacher during the school day is not recommended for this purpose.  Teachers may be out or not able to view the email in time to make the change.  We must have written documentation from the parent to make a dismissal change.  We cannot make this change with a phone call.
